Job Summary:
The Resident Care Coordinator is responsible for the overall coordination and delivery of resident care services within our facility. This role involves creating and maintaining a caring, compassionate, and safe environment for residents, while ensuring that their physical, emotional, and social needs are met. The Coordinator will work closely with nursing staff, physicians, and other healthcare professionals to develop, implement, and evaluate individualized care plans, ensuring compliance with healthcare regulations and organizational policies.

**Key Responsibilities:**
- **Care Coordination:** Lead the development and implementation of individualized care plans based on resident assessments, preferences, and needs, ensuring that care is consistent with best practices and complies with regulatory standards.
- **Staff Collaboration:** Work collaboratively with nursing staff, interdisciplinary teams, and external service providers to coordinate care, facilitate communication, and ensure that residents receive comprehensive and personalized services.
- **Resident Advocacy:** Act as an advocate for residents, ensuring their rights, dignity, and choices are respected. Address resident and family concerns in a timely and professional manner.
- **Training and Development:** Provide education and training for staff on resident-centered care practices, regulatory compliance, and specific care protocols. Ensure staff are knowledgeable about understanding and addressing the needs of diverse populations.
- **Quality Assurance:** Monitor and evaluate care delivery and outcomes to ensure quality standards are met. Participate in quality improvement initiatives and address any deficiencies in care practices.
- **Documentation:** Maintain accurate and timely documentation of resident assessments, care plans, and service delivery in compliance with organizational and regulatory requirements.
- **Communication:** Facilitate regular communication with residents and their families, keeping them informed of care plans, changes in health status, and available resources.
- **Emergency Response:** Participate in the development and implementation of emergency plans and protocols, ensuring staff and residents are prepared for any unforeseen circumstances.

**Qualifications:**
- **Education:** Bachelor’s degree in Nursing (BSN) or related healthcare field is preferred. RN or LPN licensure is required.
- **Experience:** Minimum of 2 years of experience in a healthcare setting, preferably in long-term care or geriatric nursing. Previous experience in a coordinator or management role is a plus.
- **Skills:** Strong organizational, leadership, and communication skills. Ability to build rapport with residents, families, and staff. Proficiency in electronic health record (EHR) systems and Microsoft Office Suite.
- **Knowledge:** Familiarity with resident care regulations, assessment tools, and care planning processes.
- **Certifications:** CPR and First Aid certification required. Additional certifications in Geriatric Care or Nursing Administration is an advantage.
